In concert with you, our experienced office staff will work with your insurance provider to obtain pre-authorization for your surgery. Be aware that there are not only many different insurance companies, but each company writes a wide variety of plans. Some plans will cover certain types of weight loss surgery, while other plans specifically exclude coverage of any weight loss surgery.
General Requirements
Many policies that do cover weight loss surgery will require a number of things from you, before they will even consider authorizing the surgery. The most common prerequisites include documentation of:
- Being Morbidly Obese for 3-5 years
- Failing to lose weight while participating in a 6 month physician supervised diet and exercise program
- Co-morbidities
- Psychological Evaluation
Some policies have fewer while other policies have more requirements. Many of these requirements effectively place obstacles to discourage you from persuing your need.
